TROOP 611 CONSENT FORMS

2007/2008 SCOUT YEAR

If your scout did not go to summer camp you will need to complete the Class 1 and Class 2 forms. Scouts that went to 2007 summer camp have current Class 1 and Class 2 forms on file and you will NOT need to fill out Class 1 and Class 2 forms for the upcoming year.

Please complete the following:

1. Download and complete the form EMERGENCY_CONSENT_FOR_MINORS_2007_2008.pdf. DO NOT SIGN

2. Download and printout the form titled �Personal Health And Medical Record� from the Boys Scouts Of America website: http://www.scouting.org/filestore/pdf/34414.pdf

3. Fill out the form �Personal Health And Medical Record� for Class-1 only DO NOT SIGN

4. Make six (6) copies of the the consent form (item 1 above) and three (3) of the Class 1 (annual personal health and medical history) and three (3) copies of the Class 2 (physician's evaluation form).

5. Sign each form in the areas indicated on the form. DO NOT USE BLACK INK PEN

6. Return the completed set of forms to Steve Doi (Moose Patrol) at the next scheduled scout meeting.
